UniversaCare Referral Source Map
Millen Rastogi | UniversaCare | October 2026
As of 12 Oct 2026
This page maps the practices and care teams that refer patients into home care like UniversaCare's, sorted by type, with who usually decides at each.
Most of UniversaCare's clients are families, so this is a map of referral sources rather than buyers. The groups follow what your own site describes, from hospital and rehab stays and coordination with doctors to work alongside hospice teams and care in senior communities.
Counts are for the whole US and are rough on purpose. Your New York and Connecticut service area is a slice of each row.
Where your referrals come from
Organizations by staff size, US wide. Two groups have no clean public count and say so.
| Group | Organizations in the US | Under 20 staff | 20 to 99 | 100 to 499 | 500 or more |
|---|---|---|---|---|---|
| Hospitals, including rehab and specialty hospitals | about 2,900 | not published | not published | not published | not published |
| Skilled nursing and short term rehab facilities | about 11,000 | about 2,900 | about 4,100 | about 2,900 | about 590 |
| Physician practices | about 130,000 | about 120,000 | about 12,000 | about 2,100 | about 1,000 |
| Assisted living and retirement communities | about 18,000 | about 12,000 | about 4,600 | about 1,300 | about 540 |
| Hospice and palliative care teams | no clean public count | ||||
| Medicaid long term care plans and waiver care managers | no clean public count |
US firm counts by industry code, banded deliberately.
Who usually decides on a referral
The person who usually picks the home care agency at each group, and what they need to hear from you.
| Group | Who usually decides | What they need from UniversaCare |
|---|---|---|
| Hospitals | Discharge planners, nurse case managers and social workers; the director of case management sets the preferred list | A start of care they can count on, and nursing oversight for patients going home after a stay |
| Skilled nursing and rehab facilities | The social worker or discharge coordinator; the administrator at smaller sites | Aides and nurses ready to start soon after a short term rehab stay ends |
| Physician practices | The practice manager or care coordinator; the physician on skilled orders | Updates on how the patient is doing at home, and help with medications and appointments |
| Assisted living and retirement communities | The executive director or wellness director | Extra hours of personal care or companionship for residents who need more than the community gives |
| Hospice and palliative care teams | The hospice social worker or intake lead | Aides who work alongside the hospice team for comfort and personal care |
| Medicaid long term care plans and waiver programs | The plan's care manager, or the program's case manager | An agency that takes the program and staffs the hours the plan approves |
Where the bigger referral sources sit
Hospitals and long term care plans skew large and send a steady flow, but the decision sits with a team of planners and care managers, so one contact is rarely enough. A short list per hospital, kept warm, usually works better than one big pitch.
Physician practices, assisted living communities and smaller rehab facilities are mostly small. A direct note to the practice manager or the director usually reaches the person who decides.
Where UniversaCare could start
Specialty practices, with chronic disease support
Connecticut practices, with therapy at home
Built from public federal business statistics and the company's own website, counts banded deliberately. Decision roles are typical, not named people.
